or other recreational activities 




Please read the following information carefully BEFORE you 
complete your application. 

Who should use this form? 

Use this form if you are outside Australia to apply for a visa to 
visit Australia for tourism or other recreational activities (holiday, 
sightseeing, social or recreational reasons, to visit relatives or 
friends, or other short-term non-work purposes including 
study). 

* If you intend to apply for a Sponsored Visitor (Short Stay) 
visa you should use form 48R. 

* If you intend visiting Australia for medical treatment you 
should use form 48ME. 

* If you intend to study in Australia for more than 3 months 
you should apply for a student visa. Please contact the nearest 
Australian overseas mission for information on student visa 
applications, including the correct application form. 

* If you intend visiting Australia for a short business trip 
(including attending a conference) you should use form 456. 

Types of visitor visas 

A visa must be obtained before travelling to Australia. A visitor 
visa granted following processing of this application will be in 
the form of a label, placed in your passport. 

You can apply for the following types of visa using this form: 
SHORT STAY TOURIST VISA - for a stay of up to 3 months. 

LONG STAY TOURIST VISA - for a stay of longer than 3 months 
and up to 6 months. 

If you require a stay of longer than 6 months please attach your 
reasons. 

Application Charge 

There is a non -refundable application charge which must 
accompany each application. 

* The application cannot be processed until this charge has 
been received. 

* The fee is not refundable if a visa is not granted. 

Who can you include in this application? 

Only one passport-holder can apply on one form. You can 
include in your application any children on your passport 
who will be travelling with you. If another adult is included in 
your passport they should complete a separate application form. 

How to apply 

* Complete the application form. Please note that false or 
misleading information may lead to refusal or cancellation of 
your visa, or penalties while in Australia. The completed 
application form can be lodged at your nearest Australian 
overseas mission. 

* You may arrange for another person to help you complete 
the application form, but you must sign it. If you 

have been assisted in completing the application forin, you 
should only sign the declaration^) if the information in it is 
true and correct. 



• Lodge your application form, the application charge, your 
passport and any attachments at the nearest Australian- . 
overseas mission. Your application may be lodged personally, 
by vour representative or sent by mail. 

• On the return of your passport and visa, check the details 
and conditions on the visa label carefully. If you have any 
concerns or questions about die requirements or limitations, 
you should contact the office that granted the visa before 
travelling to Australia. Ybu should not assume that any 
changes to your immigration status can be made after your 
arrival in Australia. 

Health insurance 

It is recommended that you take out health insurance for you 
and your family for the period of stay in Australia. You will not 
be covered by Australia’s national health scheme, unless you 
are covered by a reciprocal health care agreement. 

Vaccinations 

If it is your intention to enrol your child in an Australian school 
or childcare centre (creche or preschool) during your visit to 
Australia, you are strongly encouraged to cany certification of 
your child’s vaccination status. 

Vaccination is recommended against polio, tetanus, measles, 
mumps, rubella, diphtheria, pertussis (whooping cough), 
Haemophilus influenzae hypo (Hib), and Hepatitis B. 

Certification may be sought at time of enrolment. 

Note: Vaccination against rubella is also recommended for 
women of child bearing age. 

Authorisation of a person to act and receive 
communication 

Y'ou may authorise another person to act on your behalf in 
connection with your application with the Department of 
Immigration and Multicultural Affairs (DIMA). 

If so, you will need to complete form 956 Authorisation of a 
pei'son to act and receive communication which is on the last 
page of this form. 

Nominating a person to act on your behalf includes authorising 
DIMA to send to that person any communication relating to 
your application that would otherwise have been sent to you. 
You will be taken to have received any documents sent to that 
person as if they were sent to you. 

Consent to communicate electronically 

DIMA may use a range of means to communicate with you. 
However, electronic means such as fax or e-mail will only be 
used if you indicate your agreement to receiving communication 
in this way. 

Electronic communications, unless adequately encrypted, are 
not secure and may be viewed by others or interfered with. 

If you agree to DIMA communicating with you by electronic 
means, the details you provide will only be used for the purpose 
for which you have provided them. They will not be added to 
any mailing list and will not be disclosed without your consent.

Conditions for a tourist visa to Australia 

Visitors to Australia must be willing and able to abide by the 
conditions listed below while in Australia. If you are unwilling 
or unable to abide by these conditions you should not apply 
for a visa. If you do not abide by these conditions, your visa may 
be cancelled or you may be subject to other penalties. If you 
have any questions or concerns about the conditions, you 
should ask for more information from an Australian overseas 
mission. 

Conditions ' 

8101 - You must NOT work while in Australia. 

8201 - You must NOT study for more than 3 months 
while in Australia. 

8205 - Where this condition applies, and you intend to 
study for longer than 4 weeks, you are required to pass 
a chest X-ray examination before you are able to 
commence study. 

8503 - No Further Stay 

Following an assessment of your application, a visa officer may 
decide to apply the 8503 condition on your visa. The 8503 (or 
No Further Stay) condition means that the holder of the visa 
on which the condition has been imposed will not, after 
entering Australia, be entitled to be granted any other visa 1 , 
while the holder remains in Australia. 

The effect of this visa condition is that it will not be possible for 
you to apply to remain in Australia beyond the authorised 
period of stay shown on your visa label. It is particularly 
important, if your visit to Australia is to attend a specific event,

On 09/11/2001, several complaints (FD-71s) were 
taken and/jfntferviews conducted by the Newark Division with 
respect to the event surrounding the explosions at New York 
City's World Trade Center (WTC.) , the Pentagon , and airline F light 
#93 that had departed Newark. On 09/11/2001, | | 

| a housewife and Union City, NJ resident, ad vised 
that at app roximately 9:00am, 09/11/2001, I 1 neighbor, 

I called her on the telephone to tell her that she 
| saw s moke coming f rom the WTC. In order to see the WTC 
more closely, | | used 7x33mm power binoculars to view 

the scene. From the rear window of her apartment and while 
looking in the direction of the WTC, I I observed three 

(3) males kneeling on the roof of a white van located in the rear

individuals and it appeared as though they noticed her.

was also provided. | described the white van as a 

"box" -style van which had no windows on the side; it reminded

to be jovial in that they smiled, hugged one another, and gave 
"high five s." (See att ached photos) . -As the vehicle later left

provided the police with the information pertaining to the males.

Newark'^^iminal Division Agents revealed a second eyewitness who • 
reported hav ing observed t he a white van in the same parking lot

van in the parking lot as early as 8:00am on the morning of the 
explosions at WTC. 



OSJ A third interviewee later reported, as documented 
on an F©^71, having seen, on 09/11/2001 (time not noted), a "big, 
white,' model 2000 Chevrolet van, or possibly panel truck or U- 
Haul truck, bearing the words "Urban Movers" in black lettering 
along the side. The vehicle was observed in the Liberty State 
Park's Marine Terminal in Jersey City, NJ, located across the 
Hudson River from WTC. 

Pursuant to this information, East Rutherford, NJ 
police '^Snorities stopped an Urban Moving Systems van at a pre- 
positioned roadblock which was occupied by males similar to those 
described by witnesses above. The van had been traveling 
eastbound along State Route 3 in New Jersey. Following the stop, 
police detained five (5) individuals, all Israeli Nationals, on 
the grounds that they may have had information pertaining to the 
WTC explosions . The matter was later forwa rded to FBI Newark, 
and the individuals were detained based violations of immigration 
their status .

At the time of the vehicle stop, a search of the 
van and^^dft victuals was conducted. Seizure of the individuals' 
property yielded the following: a 35mm camera, several personal 

telephone -style notebooks, back packs, airline tickets with 
immediate travel dates for destinations world-wide, Israeli 
passports (in some case, expired) , a German passport, and 
thousands of dollars of cash. Student identifications later 
believed to be false were also found. Oddly, equipment typically 
used in a moving company's daily duties was not found, including 
work gloves, blankets, straps, ropes, boxes, d ollies, rolle rs, 
etc . Also not found was the video camera that | 

observed at least one of the males using to film the explosion at 
WTC. 

Focused on what might have been contained on the 
film in the video camera reportedly used by the Israeli males, ~ 
Newark's Criminal Division interviewed each of the Israeli males. 
Through interviews (and later polygraphs) , it was det ermined t hat

been observed driving a white van, bearing New Jersey plates, on 
Spirit Brook Road near Nashua, New Hampshire (just outside of 
Boston) . According to the report, the driver of the van appeared 
to be lost; the van had a sign on the side for Urban Moving 
Systems . 




H A search of Urban Moving Systems, Weehawken, NJ, 
revealeS-'i|<fre oddities which caused the search team leader to 
characterize the company as a possible "fraudulent operation." 
Little evidence of a legitimate business operation was found. 
Evidence recovery agents did seize, however, sixteen (16) 
separate computer units used by Urban Moving Systems . 

The FBI presence at the Urban Moving Systems 
search drew the attention of the local media and was later 
reported /on both television and in the local press. A former 
Urban Moving Systems employee later contacted the Newark Division 
with information indicating that he had quit his employment with 
Urban Moving Systems due to high amount of anti-American

The canvass was conducted to determine whether anybody

to them. The photocopie d photogra phs that were shown to the

pictures presented to the interviewees did not provide any names

After being advised of the official identit ies of the

provided the following information:

On 09/11/2001, woke up at approximately 7:30 

AM. She recalls the time because her husband woke her as he got 
prepared for jury duty in Newark, New Jersey (N J) . As her husband 
was showering and getting dressed, prepared the coffee 

and placed a bagel in the oven. 

Her husband went downstairs to purchase a newspaper at to 
the store located on the ground floor of the apartment complex. He 
returned and they had breakfast together. 

Her husband left for jury duty at approximately 8:25 AM 
or 8:30 AM. She then tele phonicallv contacted GEN ERAL CAB COMPANY 

for a cab to take her son, to I L

that she saw mmi rig from the WORLD TRADE CENTER (WTC) , in New 

York City. then grabbed her Binolux binoculars, 

(lightweight triple tested, 7 x 35 mm, 341 feet at 1000 yards) in 
order to see the WTC closely. t 

1 : 

recalls that she was listening to 1010 AM 

news radio station and she did not hear any information regarding 
the WTC. She recalls that the sun had risen, it was a clear day 
and there was no fog or clouds . 

Using her binoculars| looked through her rear 

window. Then she opened the balcony door and walked onto the 
balcony. As she looked in the direction of the WTC, she observed 
three (3) males kneeling on the roof of a white van in the rear 
parking lot of her apartment complex. She noticed them and it 
appeared as if they noticed her. 

| recalls that all of the males were white, 

males, in their mid- twenties, no facial hair, no glasses, "clean 
cut", no hats, all wearing short sleeve shorts, with lean builds. 

She recalls that one male (Male #1) had light hair which 
was cut very short. The top of his hair appeared somewhat "curly" 
or "wavy". He was wearing a white tee shirt and light blue jeans, 
with a hole in the left knee. He was also smoking cigarettes. She 
believes that she has the best memory of this male. 

|can not recall too many specifics regarding 

the second male (Male #2) . She believes that she has the weakest 
recollection of this male. 

recalls that the third male (Male #3) had 

dark colored, short hair which appeared straight . 

noticed that the white van on which the males 

were kneeling appeared to be a "box van " . It did no t have windows 
on the side of the van, and it reminded of a "phone 

company van" . 

The van was not parked perpendicular to the fence. 

Instead, the front of the van faced in a southeast direction. Male 
#1 knelt on the roof above the front, passenger side of the 
vehicle, and he faced the apartment complex. Male #2 knelt on the 
roof above the front, driver's side of the vehicle, and he faced

the apartment complex. Male #3 knelt on the rear portion of the 
roof of the vehicle, and he faced New York City. 

Male #3 appeared to be taking still photographs and video 
of Males #1 and #2 with the WTC in the background. All of the 
males appeared to be jovial. The smiled, they hugged each other 
and they appeared to "high five" one another. 

is certain that the first fire at the WTC had 

just started. The second explosion had not yet occurred when she 
noticed these males. There did not appear to be any other persons 
in the parking lot area. 

As the vehicle pulled away, | recorded the 

license plate on a small piece of paper contemporaneously. She 
recorded the New Jersey license plate nu mber as bei ng JRJ-13Y. She 
later provided this piece of paper to SA 

did not contact the local police department 
at the time . Instead, she waited until her husband returned from 
jury duty. She informed him of the incident arid he suggested that 
they contact the local police.

from Route three East, due to the attack on the (WTC) . During 
this work detail he received a national broadcast message related 
to the attack on the WTC which requested a “look out” for a white 
Chevrolet Van with New Jersey license plate JYJ13Y carrying 3 or 
more individuals.
